OHSAA PHYSICALS, Emergency Medical Forms and Insurance Forms
All student-athletes must have completed the OHSAA Physical Exam Form. Click here to download the OHSAA Physical Exam form. In addition to the physical form, every student-athlete must have an Emergency Medical Form and Insurance Form on file. All forms need to be filed in the Athletic Office prior to the sports mandatory practices. A copy will be kept with the athletic trainer and coaches.

CHEERLEADING UNIFORM FITTING

JR. HIGH CHEERLEADERS

FITTING FOR UNIFORMS WILL BE ON JUNE 14 @ 4PM AT THE HIGH SCHOOL

FRESHMAN/JV/VARSITY CHEERLEADERS

FITTING FOR UNIFORMS WILL BE ON JUNE 27 @ 5:30PM AT THE HIGH SCHOOL

PHYSICALS

Dr. Eric Smith will be doing physicals for $10 in the high school gym on JULY 26 from 3-6pm.
Physical forms are available on the website under athletic forms. The forms must be completed and signed by a parent in order for Dr. Smith to complete the exam. If the form is not signed, a parent must be present.
